We hereby certify that the answers on this application are true and correct. We understand that Crossroads Christian School, Inc. is a Christian organization, as put forth in its doctrinal statement. Both parents* are in agreement to home school our child(ren). We understand and agree to the following:
1. Crossroads Christian School, Inc., its directors or members, shall NOT be responsible for the education of our child(ren); rather, we are responsible for the education of our child(ren).
2. We are responsible to maintain and choose our own curriculum, and all curriculum costs are our responsibility.
3. We will meet and maintain all the requirements of and abide by the policies and procedures of Crossroads Christian School, Inc.
4. Negligence of these responsibilities may result in a penalty fee or dismissal from Crossroads Christian School, Inc. without refund and that reinstatement is dependent upon reaffirmation to these standards, a favorable decision by the Board of Directors, and repayment of applicable fees.
5. We release and hold harmless Crossroads Christian School, Inc. from any and all claims for loss, damage, and/or injury of any nature or property resulting from Crossroads Christian School, Inc. program or activities.
6. We also understand that if we are homeschooling outside the state of Florida that we may be required to fulfill obligations as stated in our state's law. It is our responsibility to become aware of these obligations and fulfill them if necessary. We release and hold harmless Crossroads Christian School, Inc. from any and all responsibility in this matter. Crossroads Christian School, Inc. does not require but does recommend that families join Home School Legal Defense Association if living outside the state of Florida.
7. We understand that every educational institution has the right to refuse any other educational institution’s records and transcripts. While this is unlikely, the possibility does exist. No educational institution whether public or private, accredited or not, can in good faith guarantee that their records will be accepted in all instances. We release and hold harmless Crossroads Christian School, Inc. from any and all responsibility in this matter.
*There may be exceptions for divorced or separated parents depending on legal custody.

We believe in the one living, eternal God, Creator of all things, who exists in three persons: Father, Son, and Holy Spirit.
We believe the Bible is the inspired, infallible Word of God, and the supreme and final authority for all faith and conduct.
We believe by the miracle of the virgin birth that the Lord Jesus Christ, eternal Son of God, became man without ceasing to be God, in order to reveal God and to redeem man. That He died for our sins as a sacrifice and rose bodily from the dead for our justification. We believe He is now exalted at the right hand of God, that He is the Head of the Church, the Lord of the individual believer, that He is ministering as our Great High Priest and Advocate and that we are awaiting His bodily return.
We believe that man was created in the image of God but fell into sin and are therefore lost. And only those who become truly born again are saved from their sins.
We believe in the resurrection of both the saved and the lost, of the saved to eternal life with the Lord, and of the lost to eternal damnation.
We believe that as Christians we are called to live a godly life and are enabled to do that by the indwelling ministry of the Holy Spirit.

Application Procedures:
Parents Responsibility:
To select and purchase curriculum for students.
Application for enrollment in CCS is open to all home-schooling households where at least one parent/guardian is committed to the Lordship of Jesus Christ. CCS does not racially discriminate and does not exclude families from enrollment based on race. However, because CCS is religious in nature, it reserves the right to make distinctions in areas of religious beliefs and affiliations.
Attendance and Reports. CCS uses a 180-day school year as required by law. However, parents may fulfill the 180 day attendance requirement any time during a twelve month period. Parents may set their own calendars. Semester reports must be submitted within six months of the time your child is enrolled and/or 90 full school days.
Testing: CCS does NOT require at this time but reserves the right to require annual testing or teacher evaluation.

When compliance with CCS requirements would result in hardship, it is the responsibility of the parent to notify the school in writing. Such a notification should be made before a violation occurs in order to work out a solution or an extension. When a violation occurs, a letter will be sent to advise of the violation and to seek resolution. If necessary, a second letter will be sent to families who fail to respond. If a family, after receiving two letters, does not respond within a period of seven (7) days from the date of the letter, the family shall be assumed to be negligent and will be notified that their child(ren) is/are removed from enrollment and the corresponding files are closed. Such action cancels all of the family's rights and privileges in CCS. All existing records shall remain on file until a transfer is requested and obligations are met. No further records will be accepted until the procedures for enrollment reinstatement are completed. Incomplete reporting/records and unpaid financial obligations will require student records to be held.
