The purpose of this form is to report credits earned at home. Students entering Crossroads in grades above ninth should utilize this form. Students who have earned high school credits below eighth grade should also include this form with enrollment.

All course titles should be specific. Do not use generalized terms such as Math, Science, Social Studies, Music, Art, etc. Specific course titles include Algebra I, Biology, American History, Piano, Drawing, etc. For one-half credit the student should have completed 60-75 hours. One credit would consist of 120-150 hours. Hours need not be counted for academic courses.

Any standardized test scores, home-school evaluations, and/or work samples should be attached to this form.
