  The following forms are necessary for enrollment or reporting to Crossroads.  Each form needs to be filled out completely.  If there is a field that does not apply to your child or family, be sure to indicate that it is not applicable with n/a.  Do not leave any fields blank.

 

All forms may be photocopied for use when enrolling in or reporting to Crossroads.  Reproduction of these copyrighted materials for other reasons is prohibited.

CCS Enrollment Packet Forms:

 

 

bulletStudent Enrollment Form (2008/2009) (2007/2008 enrollment is closed)

Used for enrolling student in Crossroads.  Each student in the family will need a separate Student Enrollment Form completed.  Returning students must also submit a new Student Enrollment Form in order for us to keep our records current.    Word

 

bullet Curriculum List -

Used for reporting curriculum choices to Crossroads.  This form is sent at enrollment or prior to the start of classes. To notify CCS of curriculum changes made throughout the year, use the CURRICULUM LIST AMENDMENT NOTIFICATION FORM below.   Word     PDF

 

bullet Statement of Faith/Policy Form -

This document should be read by parents, signed, and dated (do not alter in any way by adding to or deleting any content).  It is to be submitted annually - at the time of enrollment.  Word     PDF

 

bulletCredit Acceptance Form -

Used for requesting acceptance of previous high school credits.  Students in grades 10-12, who have not attended a public or private high school, will need to submit this form in order for their homeschool work to be accepted and awarded credits. This form is not typically needed for those entering ninth grade since there are no previous high school credits to accept. If the student has completed Pre-Algebra, Algebra I,  or other upper level Mathematics courses, Physical Science, Earth/Space Science, or other upper level Science courses in eighth grade, it would then be necessary to submit this form when entering ninth grade.  Crossroads awards high school credit for grades 9-12 (and some courses in grades prior to ninth).  Word     PDF

 

bullet Transcript Request Form -

Used for requesting child's previous school records. If the child has attended a private or public school in the past, Crossroads may request their records.  High school students may have earned credits at a previous school, which may be applied toward Crossroads' requirements.  Complete this form and submit it with enrollment . NOTE: Parents may also contact the transfer school Guidance Dept directly to request  that the records be forwarded.  Word     PDF  

 

 

CCS Reporting Packet Forms:

 

 

bullet Semester Reporting Form - (2007/08)

This form is used to report bi-annual or semester reporting after the completion of 90 days of schooling.

Samples/Testing and Non-Traditional Credit Worksheet(s) (if necessary) should accompany this form. 

 

bullet Annual Reporting Form -(2007/08)

This form is used to report annual reporting after completion of all 180 days of schooling. 

Samples/Testing and Non-Traditional Credit Worksheet(s) (if necessary) should accompany this form.

 

bullet Non-Traditional Credit Worksheet -

This form is used to record (log) credit hours for courses such as PE, Drama, Work Experience, etc that are not textbook-based.  It should be sent along with reporting in order for CCS to determine the proper credit amount(s) to award.

 

bullet Reporting Schedule -

This is a document describes the method in which to report to Crossroads.  It does not need to be returned to CCS.  Word     PDF

 

 

CCS Information Request Forms:

 

 

bulletFormal Request For Transcript -

This form should be completed and mailed to Crossroads for the purpose of requesting a certified transcript.  Certified transcripts are mailed directly to other institutions.  In order to maintain the integrity of our records, transcripts are not mailed to parents or students.  Transcripts may take up to four weeks for processing (due to the uniqueness of the school).  Transcripts needed in less time will be processed only as time allows.  Requests should be done via this form and not email or phone calls.  Institutes may request transcripts from Crossroads directly. NOTE: Crossroads Christian School will process up to four transcripts at no charge.  After the initial four, the cost will be $5.00 per transcript, paid in advance.  Word

 

bullet Diploma Request Form -

This form should be used to request a diploma prior to graduation from Crossroads Christian School.  Note: This form should be sent at the end of first semester or at the time the student has completed half his/her senior year.  It should not be sent at the time of enrollment.  Word

 

bullet High School Credit Statement Request Form -

This form is used to request a credit statement which indicates credits required, earned, and remaining for CCS graduation.  Only one (1) request per academic year is permitted. (We suggest the request be made after final reporting has been sent).

 

 

CCS Information Change Notification Forms:

 

 

bullet Curriculum List Amendment Notification -

Used for reporting any curriculum changes made throughout the year (this form can be sent to our office anytime during the year).  *It is very important to notify CCS of curriculum changes as quickly as possible* How to use the form:  Indicate whether you are dropping or adding the course/curriculum by checking the appropriate box. 

 

If dropping the course completely, indicate DROP and place the information in the appropriate areas. If adding a new course (without dropping another), just indicate ADD and place the information in the appropriate areas.  If dropping a course and adding another course in it's place, you will indicate DROP and place the course information to be removed from the list and in the next section, indicate ADD and provide the replacement curriculum information.

 

bullet Personal Information Update Form -

Used for reporting changes/updates in personal contact information to CCS.  This form may be mailed, e-mailed (as an attachment), or faxed to our Records Department. *It is very important to notify CCS of personal information changes as quickly as possible*

 

 

 
      Downloading
  For those with Microsoft Word or Adobe Acrobat Reader, you may download the necessary forms and store on your hard drive.

 

 

      Printing
  For those without Microsoft Word or Acrobat Reader, click on the document's title to print.  Some forms print best in the landscape format.  Adjusting one's screen resolution may be necessary if forms appear irregular.
