
We've now made reporting to Crossroads Christian School easier for our
families. It is our hope that the new reporting schedule will give our families
the freedom they enjoy while still holding them accountable. If you have any

At Registration:
All forms should be filled out completely and signed by both parents or as
specified.
- Student Enrollment Form
- Curriculum List
- Records Transfer Request (if child has attended a private or public school
in the past)
- The Family Registration Fee and Tuition for each student
- Birth Certificate and shot record (or signed religious waiver) if child
has never attended a school previously.
- If child has not attended school in the past and is at the high school
level, we will need a list of subjects, grades, curriculum, and a copy of
evaluations/test scores in order to give your child high school credit for
previous grade levels (grades 9-12 only).
End of First Semester-(within 6 months of enrollment or 90 full
school days)
- Attendance and Grade Sheet - should reflect at least 90 days of
attendance, and all grades should be shown as well semester average.
- Curriculum List - If curriculum has changed since the one submitted at
enrollment.
- Copy of lesson plans or logs/journals.
If parent submits yearly standardized test scores, lesson plans do not have to
be submitted. CCS must be notified at the beginning of the year if you choose
this option. If you change your mind and do not submit the test scores, you
will be responsible for that year's lesson plans and such.
- One sample of the child's work or one test for each subject should be
submitted for each month.
- Optional: reading list, field trip list, recognition/awards, community
service, etc.
End of Second Semester - (within 6 months or 90 days from the end of
the first semester)
- All the requirements shown above.
- Enrollment forms, Family Registration Fee, and Tuition for the next school
year.
